About Us

Home and Community Care Services is dedicated to supporting adults and elderly Kahnawa’kehró:non, empowering their independence and enhancing their quality of life within the comfort of their home. Our services are delivered through a multidisciplinary team of healthcare professionals including a support counsellor, case workers, homecare workers, and services provided through Kateri Memorial Hospital Centre (KMHC) and CISSSMO.

What We Offer

Our services are designed to cater to a wide range of needs, from managing minor health concerns, providing compassionate support, offering practical assistance and delivering personal care. Each client receives a personalized care plan tailored to their unique situation. Services may include home care nursing, social support, meals on wheels, and assistance with daily activities.

Adults & Elders Support Counsellor

The Adult & Elders Support Counsellor is here to provide guidance, assistance and act as a liaison for various essential applications and documents. Whether you need help navigating federal or provincial systems, the counsellor is available to support you with the following:

  • Pension applications
  • Federal old age security application, including Guaranteed Income Supplement (GIS)
  • Quebec Pension Plan (QPP)
  • Social Security Administration (SSA) Documents
  • Regie Assurance Maladie du Quebec (RAMQ) Quebec Medicare, which is limited to the following:
    1. Certification of residency affidavits for lost or expired Quebec Medicare cards
    2. Application for reimbursement of medical costs outside of Quebec
  • Civil Status applications including birth, marriage, and death certificates
  • Wills (clients will one copy)
  • Living will & mandates
  • Power of Attorney (POA) 

Adults & Elders Case Workers

Case Workers in Home and Community Care are members of the Quebec Order of Social Workers, dedicated to supporting individuals and families in managing various aspects of their lives. They play a crucial role in improving clients’ quality of life through compassionate support and practical assistance.

Their services include:

  • Assessing the needs, strengths, and challenges of individuals and families to develop personalized care plans
  • Collaborating with community partners, such as Kateri Memorial Hospital and external hospitals, for appropriate discharge plans
  • Emotional support and counselling to assist individuals and families coping with difficult situations such as illness, disability, or grief
  • Connecting clients with community resources including healthcare providers, financial assistance programs, and support groups
  • Advocating for clients' rights and helping them navigate complex systems such as healthcare, housing, and legal services
  • Providing education on health, safety, and self-care practices to promote the well-being of clients and their families
  • Coordinating care with other healthcare professionals such as nurses, doctors, and therapists
  • Crisis Intervention and support as clients manage emergencies such as sudden illness, family conflicts and mental health crises 

Home Care

The Home Care team provides personal care to the clients of HCCS, ensuring their needs are met with professionalism and compassion. Case workers assess individuals’ requirements and develop a personalized care plan to their unique situation. The team of Home Care workers collaborates closely with Home Care nurses to ensure patient centred care are effectively addressed. Home Care services are available to Kahnawa’kehró:non seven days a week, including holidays, from 7:00 AM to 8:30 PM. 

The core responsibilities of our Home Care team are:

  • Personal and supportive care
  • Medication Assistance (assist clients from dispills only)
  • Domestic/Housekeeping (i.e., household management & laundry)
  • Meal preparation (in clients' homes)
  • Monday-Friday delivery of Meals on Wheels
  • Respite / needed psychosocial interactions
  • Escorting clients to doctor appointments and follow ups
  • Providing transportation to and from clients' home
  • Accompanying clients on errands (i.e.., bank, post office, groceries)  

Turtle Bay Elders Lodge

The Turtle Bay Elders lodge is in the same facility as Home and Community Care Services. It is a 22-room facility exclusively for Kahnawa’kehró:non, who are seeking a safe, quiet, and welcoming environment to live in.

We work in collaboration with various departments within KSCS along with Kateri Memorial Hospital Centre and CISSMO to deliver comprehensive and accessible home and community care services.
