Assisted Living Services Survey
Individuals with Special Needs/Disabilities and their Caregivers
We would like to thank you in advance for taking the time to complete this survey. Assisted Living Services (ALS) is conducting this survey to get a picture of the number of Kahnawákeró:non who have disabilities, and what types of needs they have. , We also would like to get a picture of who caregivers of individuals with special needs are. We are interested in Kahnawákeró:non of all ages (children, teens, adults, and elderly). This effort is part of the Kahnawake Shakotiia'takehnhas Community Services (KSCS) Strategic Plan and will help us obtain appropriate resources, identify gaps and develop services based on needs. Your answers to this survey are very important and will help us do that.
The survey will be open until the end of May, 2018.
The information you provide on this survey is confidential, so you do not need to tell us your name or the names of anyone living in your house.
In addition to gathering information through a survey, we are also interested in speaking with families to find out about their experiences. If you would like to be contacted for a discussion, please write your name and telephone number in the space provided at the end of the survey and someone will contact you.
The term special needs or disability refers to any condition (physical, emotional, behavioral, or learning disability or impairment) that causes an individual to require additional or specialized services or accommodations.
The term support refers to anything the individual needs to help them, including services or equipment.

Note:
- It is very important that you answer the questions based on the people who live in your house full-time so that we do not count individuals more than once.
- There may be more than one individual in your household with a disability, or the individual may have more than one caregiver. If this is the case, in Part 2 of the survey, please fill out information for each individual and each caregiver.
